When you start mechanical ventilation, you want to see how the patient’s blood gases respond to the new support. Drawing an arterial blood gas about 15 minutes after initiation gives a timely and reliable picture of how well the ventilator settings—FiO2, PEEP, and ventilation rate—are supporting gas exchange. By this point, the alveolar–arterial gas diffusion and circulatory equilibration have occurred enough to reflect the true effect of the ventilator changes.

If you measure too soon, within just a few minutes, the results may not reflect the full impact of the new settings and can be misleading. If you wait much longer, you delay opportunities to adjust therapy and may risk ongoing hypoxemia or hypercapnia. So, 15 minutes is the practical window to reassess and fine-tune the ventilator.
